These files are Microsoft Word forms. You should be able to enter text by typing to complete each field, then using the tab key or your mouse to move from field to field, and by using the spacebar or your mouse to select the checkboxes. Depending on the settings of your computer, clicking the file will either download it or open it for completion in your browser.

 

To email the completed form you must save the file before you send it.
